Use this form to request a licence for the use of the Quality of Life questionnaires.<\/p>\n

If the questionnaires are used only for routine clinical purposes<\/strong>, there is no need to seek permission and no need to apply for a licence. For all other purposes you will need to apply for a licence.<\/p>\n

Licences may be granted without charge to not-for--profit institutions and to academics for non-commercial purposes only. If you are using the questionnaires for any other purpose (including clinical trials) you will need to apply for a licence with a charge. If you need any help with licensing, please contact technologytransfer@cardiff.ac.uk<\/a>.<\/p>\n
