Use this form to request a licence for the use of the Quality of Life questionnaires.
If the questionnaires are used only for routine clinical purposes, there is no need to seek permission and no need to apply for a licence. For all other purposes you will need to apply for a licence.
Licences may be granted without charge to not-for--profit institutions and to academics for non-commercial purposes only. If you are using the questionnaires for any other purpose (including clinical trials) you will need to apply for a licence with a charge. If you need any help with licensing, please contact technologytransfer@cardiff.ac.uk.

Students should not request a licence directly.
Please ask your institution to request a licence on your behalf. This means that it should not be you, as a student, who requests the licence. The licence should be requested by a member of staff of your institution, college or university. Please seek advice from your research or medical supervisor over the on-line completion of the licence request. This is because it is important that the institution takes responsibility for the licence, not the student.
The authorised staff member should choose the option "Study or use not by a with-profit company/organisation or funded by a with-profit company/organisation" and complete the form.
